Job summary

The role requires a BSN, RN license in the state or compact, and BLS. At least 1 year of clinical experience is required, with 3 years preferred; strong documentation and communication are essential.

Qualifications

• BSN required or preferred depending on site.
• BLS Basic Life Support required.
• RN license in the state or by compact.
• ACM/ CCM / ANCC Nursing Case Management board certification preferred.

Responsibilities

• Identifies and prioritizes patients in need of care management using holistic factors; plans with patient and team for optimal outcomes.
• Completes documentation in the patient record; ensures clear, timely handovers at care transitions.
• Ensures plan of care reflects patient and caregiver goals for post-hospital planning.
• Follows standardized practices for Advance Care Planning, length of stay management and readmission prevention.
• Supports timely testing and treatment by addressing barriers to progression of care.
• Promotes appropriate resource utilization and interdisciplinary discharge rounds.
