Job summary

Bon Secours in Richmond, VA seeks an RN Care Manager for a weekend PRN role (~20 hours/week). The RN Care Manager coordinates care for patients, assesses needs including social determinants, and plans with the healthcare team to ensure seamless transitions.

Responsible for documentation, discharge planning, and evaluating care plans across transitions. Required: RN license in Virginia, BLS certification, and 1 year in a clinical setting; BSN preferred.

Qualifications

• Bachelor of Science in Nursing (preferred for BSMH, required for RSFH)
• RN License in the state in which they are working or covered by compact
• Accredited Case Manager Certification (ACM) or CCM or ANCC Nursing Case Management board certification (preferred)
• 1 year of experience in clinical setting (required)
• 3 years in acute care (preferred)
• Ambulatory or post-acute care coordination experience (preferred)

Responsibilities

• Identifies and prioritizes patients in need of care management services with holistic approach
• Completes all necessary documentation with clear, concise notes
• Documentation reflects plan of care for post-hospital needs
• Follow standardized practices related to Advance Care Planning, Length of Stay management and readmission prevention
• Supports denial prevention and removes barriers to progression of care
• Promotes proactive care and ensures resources used appropriately
