Job summary

Qualifications

• Licensure to practice nursing in state or compact jurisdiction.

Responsibilities

• Coordinate care for patients to support safe, timely transitions across care continuum.
• Identify and assess patients needing active case management using biopsychosocial and other factors.
• Document care plans clearly and ensure handovers during level changes.
• Promote Advance Care Planning and discharge planning processes.
• Collaborate with interdisciplinary team to optimize resource use and reduce readmissions.
• Promote timely testing and treatment by addressing barriers to progression of care.
