Job summary

Mercy is seeking a Care Manager to join the interdisciplinary team. This role assesses, plans, advocates, and coordinates patient care from admission to discharge to ensure a safe transition after hospitalization.

You will work within Mercy's mission and values, using performance metrics and best practices to improve transitions, length of stay oversight, and patient safety. Relevant MSW education and licensure are required.

Qualifications

• Master's in Social Work or MSW candidacy awaiting conferment.
• Social Workers hired after 12/25/2011 must meet MSW requirements.
• 0–2 years' experience in acute care hospital setting.

Responsibilities

• Assess, plans, advocates, and coordinates care from admission to discharge.
• Ensure safe transition post hospitalization and maintain patient experience, safety, and quality of care.
• Operate within Mercy mission, values and service standards.
