Job summary

Wellstar Cobb Hospital is hiring a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. The role engages patients and families to plan progressing care, manage length of stay, and promote efficient resource use.

Collaborate with physicians, the care team, and social work to remove barriers to discharge and arrange post-acute referrals while documenting care progress in the EMR.

Qualifications

• Associates degree or nursing diploma required; bachelor’s preferred.
• Active RN license in the single-state or compact multi-state.
• BLS certification required.

Responsibilities

• Assess patient needs and coordinate transitional care plans.
• Collaborate with physicians, care team, and family.
• Lead discharge planning and ensure timely transitions.
• Document care progression and outcomes in EMR.
• Identify barriers to discharge and coordinate referrals.
