Job summary

Wellstar Cobb Hospital seeks a Care Coordinator RN to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure care needs are met and discharge planning is optimized.

As part of the Cobb Hospital Care Coordination team, you will collaborate with physicians, nurses, social workers, and care managers, facilitate care conferences, and support efficient utilization of resources while focusing on quality patient outcomes.

Qualifications

• RN license required (single state or compact)
• BLS certification required
• Associates or Diploma in Nursing; Bachelor’s preferred

Responsibilities

• Assess psychosocial and functional status
• Plan transitional care and discharge
• Coordinate clinical care progression
• Facilitate patient/family care conferences
• Participate in interdisciplinary rounds
• Educate patient and family on care plan
• Identify and resolve discharge barriers
