Job summary

Wellstar Cobb Hospital is seeking an RN Care Coordinator for a full-time day shift onsite. You will assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure care needs are met.

Collaborating with physicians and the interdisciplinary care team, you will support care progression and discharge planning, document in the EMR, and work to remove barriers to timely discharge while ensuring quality outcomes.

Qualifications

• Associate's Degree in Nursing from an accredited school.
• Georgia RN license required and active.
• All certifications are required upon hire unless otherwise stated.
• Reg Nurse (Single State) or RN - Multi-state Compact.
• Basic Life Support (BLS) certification.
• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication skills, with ability to work under pressure.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage with patient and family to assure care needs are met and plan for discharge.
• Collaborate with physicians and interdisciplinary care team to support progression of care and discharge planning.
• Document care plans and progress in the electronic medical record and track discharge milestones.
