Job summary

Wellstar Health System is hiring an RN Care Coordinator for Cobb Hospital in Georgia. This full-time day position involves assessing transitional care needs, coordinating across the continuum, and engaging with patients and families to ensure care goals are met.

You will work with the care team to promote efficient discharge planning and resource utilization. Required is an Associate's Degree in Nursing and a Georgia RN license, with at least one year of acute care nursing experience.
