Job summary

Wellstar Health System is hiring a Registered Nurse Care Coordinator for a Full-time Day shift at Cobb Hospital in Georgia. The coordinator assesses transitional care needs, plans care across the continuum, and engages patients and families to ensure discharge goals are met.

The role emphasizes collaboration with physicians and the care team, managing length of stay, and optimizing utilization of resources. Previous hospital experience is required.

Qualifications

• Associate's Degree in Nursing from an accredited school of nursing is required.
• Georgia RN License required, and multistate compact preferred.
• Excellent written and verbal communication skills are essential.
• Ability to collaborate with physicians, care team, patient and family.

Responsibilities

• Assessment and discharge planning to ensure timely, safe transitions.
• Coordinate care across the continuum with multidisciplinary teams.
• Engage with patient/family to align goals and care plans.
• Identify barriers to discharge and coordinate with payers and social work.
• Document care plans, progress, and outcomes in the EMR.
• Support post-acute referrals and follow-up as needed.
