Job summary

Wellstar Cobb Hospital in Georgia seeks a Care Coordinator RN to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure goals are met.

The RN will work with the care team to plan discharge, manage length of stay, and promote efficient resource use, while collaborating with physicians and social work to address barriers and optimize outcomes.

Qualifications

• RN nursing license required with acute care experience.
• Minimum 1 year nursing experience in acute care settings.
• All certifications required upon hire unless stated otherwise.

Responsibilities

• Assess chronic disease management needs and psychosocial risk factors.
• Coordinate discharge planning and ensure timely progression to discharge.
• Participate in interdisciplinary rounds and family education.
• Document care plans and monitor progress in medical records.
• Collaborate with physicians, care team, and utilization review.
