Job summary

Wellstar Health System is seeking an RN Care Coordinator for Wellstar Cobb Hospital to assess transitional care needs and coordinate across the care continuum, guiding patients and families through discharge and progression.

You will partner with physicians, care teams, and utilization review nurses to remove discharge barriers, ensuring safe and efficient transitions and contributing to improved patient outcomes.

Qualifications

• Active RN license required.
• Minimum of 1 year of nursing experience in an acute care setting.
• Current BLS certification.

Responsibilities

• Assess transitional care needs and coordinate across care continuum.
• Support care progression and safe hospital-to-home transitions.
• Collaborate with physicians, utilization review nurses, interdisciplinary care teams, patients, and families to address care needs and remove discharge barriers.
