Job summary

Wellstar Cobb Hospital is seeking an experienced Care Coordinator RN to assess transitional care needs, coordinate across the care continuum, and engage with patients and families to meet care goals. The role emphasizes discharge planning, effective communication, and collaboration with physicians and the care team to promote timely, efficient transitions.

Qualifications

• 1 year nursing experience in acute care setting required.
• Excellent written and verbal communication skills.
• Knowledge of local/state/federal regulations and community resources.

Responsibilities

• Assess chronic disease management and psychosocial risk factors; plan discharge.
• Coordinate with PAS/financial counselor/UM nurse for insurance needs.
• Collaborate with physicians and care team for care progression and discharge plans.
• Participate in interdisciplinary rounds and patient/family education.
• Identify barriers to discharge and facilitate timely disposition.
• Document all services in the medical record and track avoidable days.
