Job summary

Wellstar Cobb Hospital in Cobb County, GA, seeks a Care Coordinator RN to assess transitional care needs, coordinate across the care continuum, and engage patients and families to ensure discharge planning is efficient and patient-centred.

The ideal candidate has at least 1 year of acute care nursing, strong communication and collaboration skills, and a commitment to coordinating with physicians, social work, and payers to achieve timely, high-quality transitions.

Qualifications

• Associates Degree in Nursing or Diploma required; BSN preferred.
• RN license required at hire (state licensure).
• Minimum 1 year nursing experience in acute care.
• BLS certification or equivalent required.

Responsibilities

• Assess patients' transitional care needs and psychosocial factors.
• Plan and coordinate discharge and transitions of care.
• Collaborate with physicians, social workers and care teams.
• Document care plans and progress accurately.
