Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN to assess transitional care needs and coordinate services across the care continuum. You will engage patients and families to ensure discharge planning and care progression align with goals, while working with interdisciplinary teams to optimize resources and outcomes.

The role requires a minimum of 1 year of acute care nursing experience, RN licensure, BLS, and strong communication skills.

Responsibilities
