Job summary

ADAPT Community Network in New York City seeks a Residence Program Specialist to support the care, development, and independence of individuals in our residential facilities. You will promote dignity, respect, and a homelike environment while delivering direct support, skill-building, and advocacy to help residents achieve personal goals.

Reporting to the program director, you will collaborate with an interdisciplinary team, follow AMAP protocols, and maintain accurate records, ensuring safety

Qualifications

• High School Diploma or GED required.
• Six months of related experience preferred but not required.
• AMAP certification required at hire or within six months.
• Valid New York State Driver’s License.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills.
• Ability to respond to crisis situations and make sound decisions.
• Commitment to confidentiality and safeguarding rights.
• Ability to work independently and as part of a team.

Responsibilities

• Provides care and supports the people we support in achieving greater independence and personal development.
• Ensures the safety, health, and well-being of the people we support at all times.
• Teaches and assists with activities of daily living (ADLs) and community living skills.
• Documents daily activities, progress notes, incident reports, and other records accurately and timely.
• Responds to crisis situations and medical emergencies, including first aid.
• Administers medications as AMAP under nursing supervision.
• Provides transportation to programs, work, appointments, and activities.
• Assists with meal planning, grocery shopping, and meal preparation.
• Participates in housekeeping, laundry, and light maintenance.
• Attends staff meetings, trainings, and in-service sessions.
