Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support residents in a NY residential facility. You will provide direct care, foster independence, and participate in treatment plans to promote dignity and safety.

Responsibilities include ADL instruction, documentation, crisis response, medication administration under supervision, transportation, and meals. A high school diploma and AMAP eligibility are required; compassionate teamwork is essential.

Qualifications

• High School Diploma or GED required.
• Six months of related experience preferred.
• AMAP certification at hire or within six months.
• Valid New York State Driver’s License.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills.
• Ability to respond to crisis situations and make sound decisions under pressure.
• Commitment to confidentiality and safeguarding rights.

Responsibilities

• Provide direct support and promote independence for residents.
• Ensure safety, health, and well‑being of residents at all times.
• Teach ADLs and daily living skills.
• Maintain a homelike, respectful atmosphere in the residence.
• Document daily activities, progress notes, and reports accurately.
• Respond to crises and medical emergencies; contact emergency personnel as needed.
• Administer medications as AMAP under nursing supervision.
• Transport residents to program, work, and appointments.
• Support meal planning and nutrition; assist with shopping and cooking.
• Participate in meetings, trainings, and quality care initiatives.
