Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support residents in ADAPT residential facilities. You will promote independence, safety, and a homelike environment while delivering direct support, skill-building, and advocacy to help participants reach personal goals.

You will teach activities of daily living, document progress, and collaborate with an interdisciplinary team. Strong communication and crisis response skills are essential; a NY driver’s license and AMAP

Qualifications

• High School Diploma or GED required.
• Ability to read, write, speak and understand English.
• AMAP certification or eligibility within six months of hire.
• Valid NY driver’s license in good standing.

Responsibilities

• Provide direct support and skill-building to individuals in residential settings.
• Ensure safety, health, and well-being of residents at all times.
• Assist with activities of daily living and independence skills.
• Document daily activities, progress notes, and incidents accurately.
• Administer medications as AMAP under nursing supervision.
• Attend staff meetings and participate in treatment plans.
