Job summary

Adapt Community Network is seeking a Residence Program Specialist to support residents in a New York residential facility. You will provide direct care, assist with ADLs, and help residents build skills in a safe, respectful environment.

The role emphasizes teamwork with clinical staff, documentation, and crisis response, with responsibilities including medication administration under supervision and facilitating independence for residents.

Qualifications

• High School Diploma or GED required.
• AMAP certification within six months of hire.
• Valid New York State Driver’s License in good standing.
• Ability to read, write, speak and understand English; strong communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide care and supports to individuals to foster independence and personal development.
• Ensure safety, health, and well‑being of residents at all times.
• Teach activities of daily living (ADLs) and daily living skills.
• Maintain a personalized, homelike, respectful atmosphere in the residence.
• Participate in interdisciplinary treatment plans and implement goals.
• Document daily activities and incident reports accurately and timely.
• Respond to crisis situations and medical emergencies; provide first aid when needed.
