Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support individuals in a residential facility. The role emphasizes dignity, safety, and independence, with hands-on care, ADL support, and advocacy to help people reach personal goals and enjoy a high quality of life.

Under program director guidance, the specialist conducts daily documentation, crisis response, and medication administration under nursing supervision, while promoting recovery, respect, and community integration.

Qualifications

• High School Diploma or GED required.
• AMAP eligibility within six months of hire.
• Valid NY Driver’s License in good standing.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills.
• Ability to work independently and in a team.

Responsibilities

• Provides care and supports residents toward greater independence and personal development.
• Ensures safety, health, and well-being of residents at all times.
• Teaches ADLs and community living skills; fosters a respectful, homelike atmosphere.
• Participates in interdisciplinary treatment plans and implements goals.
• Documents daily activities, progress notes, and incident reports accurately.
• Responds to crises and medical emergencies; provides first aid when needed.
• Coordinates with emergency personnel and maintains safety of residents.
• Ensures compliance with abuse prevention and reporting policies.
• Administers medications as AMAP under nursing supervision.
• Provides transportation to programs, work, appointments, and activities.
• Assists with meal planning, shopping, and meal prep for nutrition and independence.
• Supports housekeeping, laundry, and light maintenance; maintains a safe environment.
• Attends meetings and trainings to support ongoing development and quality care.
• Promotes dignity, respect, and person-centered approaches in all interactions.
