Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support residents in a ADAPT residential facility. You’ll help with ADLs, safety, advocacy, and skill-building to promote independence and quality of life.

Responsibilities include documenting progress, coordinating with care teams, responding to emergencies, administering AMAP under supervision, and assisting with meal planning and transportation. A NY driver’s license and strong communication are required.

Qualifications

• High School Diploma or GED required.
• Six months of related experience is preferred but not required.
• AMAP certification at hire or within six months.
• Valid New York State Driver’s License in good standing.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills for working with residents, families, coworkers, and partners.
• Ability to respond to crises and make sound decisions under pressure.
• Commitment to confidentiality and safeguarding residents' rights.
• Ability to work independently and as part of a team.

Responsibilities

• Provides care and supports the people we support in achieving greater independence and personal development.
• Ensures the safety, health, and well-being of the people we support at all times.
• Teaches and assists with activities of daily living (ADLs) and living skills.
• Maintains a personalized, homelike, respectful atmosphere within the residence.
• Documents daily activities, progress notes, and required records accurately and timely.
• Responds to crisis situations and medical emergencies, including first aid.
• Contacts emergency personnel when necessary to ensure safety.
• Administers medications as AMAP under nursing supervision.
• Provides transportation for program, work, appointments, and recreation.
• Assists with meal planning, grocery shopping, and meal preparation.
• Participates in housekeeping, laundry, and light maintenance to maintain a safe environment.
• Attends staff meetings and trainings to support development and quality care.
• Maintains confidentiality of all resident information and documentation.
