Job summary

ADAPT Community Network is seeking a Residence Program Specialist in New York to support individuals in ADAPT residential facilities. You will assist with daily living skills, safety, and advocacy to promote independence and quality of life.

The role requires a High School Diploma or GED, NYS driver’s license, and AMAP eligibility. You will collaborate with a multidisciplinary team, document progress, and respond to emergencies while maintaining confidentiality.

Qualifications

  • High School Diploma or GED required.
  • AMAP at time of hire or within six months of date of hire.
  • Valid New York State Driver’s License in good standing.
  • Ability to read, write, speak and understand English.
  • Strong interpersonal and communication skills for working effectively with people we support, families, coworkers, and community partners.
  • Ability to respond appropriately to crisis situations and make sound decisions under pressure.
  • Commitment to maintaining confidentiality and safeguarding the rights of the people we support.
  • Ability to work independently and as part of a team.

Responsibilities

  • Provides care and supports individuals in achieving greater independence and personal development.
  • Ensures safety, health, and well-being of individuals at all times.
  • Teaches activities of daily living (ADLs) and community living skills.
  • Maintains a personalized, homelike, respectful atmosphere in the residence.
  • Participates in interdisciplinary treatment plans and implements program goals.
  • Documents daily activities, progress notes, and required records accurately and timely.
  • Responds to crises and medical emergencies, providing first aid and contacting emergency personnel as needed.
  • Contacts emergency personnel when necessary to ensure safety.
  • Follows OPWDD-approved behavior management guidelines and uses approved interventions when required.
  • Administers medications as AMAP under nursing supervision.
  • Provides transportation to programs, work, appointments, and activities.
  • Assists with meal planning, grocery shopping, and meal preparation.
  • Performs housekeeping, laundry, and light maintenance to maintain a safe environment.
  • Attends staff meetings and trainings to support ongoing development.
  • Maintains confidentiality of resident information.
  • Performs other related duties as assigned.
