Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support residents in a residential facility. You will foster dignity, independence, and a homelike atmosphere while assisting with daily living skills and treatment plans.

Responsibilities include documenting progress, responding to crises, administering medications as AMAP under supervision, and coordinating transportation and meals to promote nutrition and independence. A NY driver’s license is required.

Qualifications

• High School Diploma or GED required.
• Six months of related experience is preferred but not required.
• AMAP certification at hire or within six months.
• Valid New York State Driver’s License.

Responsibilities

• Provides direct support and skill-building to residents.
• Ensures safety, health, and well-being of residents at all times.
• Teaches activities of daily living and maintains a homelike, respectful atmosphere.
• Documents daily activities, progress, and incidents accurately and timely.
• Responds to crises and medical emergencies; contacts emergency personnel as needed.
• Administers medications as AMAP under nursing supervision.
