Job summary

ADAPT Community Network seeks a Residence Program Specialist to support individuals living in a residential setting, promoting independence and dignity. You will provide direct care, assist with daily living activities, and help maintain a safe, respectful home environment under supervision.

The role involves documenting progress, participating in treatment plans, and responding to emergencies, while ensuring confidentiality and adherence to agency policies.

Qualifications

• High School Diploma or GED required; six months related experience preferred but not required.
• AMAP (Medication Administration Personnel) approved at hire or within six months.
• Valid New York State Driver’s License in good standing.
• Ability to read, write, speak and understand English.

Responsibilities

• Provides direct support and advocacy to individuals in a residential facility.
• Teaches activities of daily living and promotes independence.
• Maintains a safe, homelike, respectful atmosphere.
• Documents daily activities, progress notes, and incidents accurately and timely.
• Responds to crisis situations and medical emergencies and reports concerns per policy.
