Job summary

Citizens Options Unlimited in New York is seeking a Direct Support Professional to provide ongoing guidance to individuals with disabilities in daily living activities, health, safety, and social participation. The role supports people in developing independence within their routines and community involvement.

Responsibilities include assisting with self-care, preparing meals, transportation, and following treatment plans.

Qualifications

• High school diploma or equivalent preferred.
• Must submit a valid NYS drivers' license, and maintain a qualified driver status throughout employment.
• Must be able to communicate and understand written and oral directives/information

Responsibilities

• Provide ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life.
• Assist with meals, health, safety, self-care routines, and household chores to promote independence.
• Provide transportation to medical appointments, shopping, recreation, and other activities.
• Follow treatment plans and document progress in agency records and related materials.
• Mentor new Direct Support Staff and participate in inter-disciplinary team meetings.
