Job summary

Citizens Options Unlimited is seeking a Direct Support Professional to support individuals with disabilities in a residential services setting in New York. You will assist with daily living activities, health and safety, meal preparation, and community engagement, while following individualized plans and documenting progress.

The role requires strong communication, reliability, and empathy, with a focus on promoting independence and well-being for residents.

Qualifications

• High school diploma or equivalent preferred.
• Must submit NYS driver's license and maintain qualified driver status.
• Must be able to communicate and understand written and oral directives/instructions.

Responsibilities

• Provides ongoing guidance and support to individuals to promote participation in daily life.
• Supports individuals in becoming active community members and maintains natural supports.
• Ensures a safe and clean home by assisting with household chores as needed.
• Follows treatment plans and uses adaptive equipment to promote health and safety.
• Assists with meal preparation, including diet adherence and portion control.
• Assists with medication administration according to AMAP policy.
• Documents treatment plans and progress notes to support independence and well-being.
• Attends inter-disciplinary team meetings and participates in staff meetings.
• Provides transportation to medical appointments, shopping, and activities.
• Mentors new Direct Support Staff by sharing routines and training materials.
• Performs other duties as requested by supervisor.
