Job summary

Qualifications

• High school diploma or equivalent preferred.
• Must submit a valid NYS driver's license and maintain qualified driver status.
• Must be able to communicate and understand written and oral directives.

Responsibilities

• Provide ongoing guidance and support to individuals with intellectual and other disabilities to promote independence in daily life.
• Assist people supported with self-care, health, safety, meals, and activities to enhance quality of life.
• Transport individuals to medical appointments, shopping, and recreational activities as needed.
• Follow treatment plans, document progress, and participate in inter-disciplinary team meetings.
• Mentor new staff and support orientation and training of team members.
• Maintain safety and cleanliness of living environments and ensure adherence to policy.
