Job summary

Citizens Options Unlimited seeks a dedicated Direct Support Professional to help people live active, independent lives in their homes and communities. You will support daily activities such as self-care, meals, transportation, and safety while following individualized plans and documenting progress.

Colleagues collaborate with nurses and supervisors to ensure quality care, safety, and wellbeing. The role requires a NYS driver’s license and willingness to complete AMAP training within six months

Qualifications

• High school diploma or equivalent preferred.
• Qualified NYS driver's license required and maintained throughout employment.
• Ability to communicate and understand written and oral directives.
• AMAP Training is required and must be completed within six months of hire.
• First Aid/CPR/SCIP-R Training is preferred.

Responsibilities

• Ensure a safe and clean home by supporting household chores and participating as needed.
• Follow treatment plans and use adaptive/safety equipment to promote independence and wellbeing.
• Assist with meal preparation, teaching diet adherence and portion control.
• Assist with medication administration according to AMAP policy.
• Coordinate with residential nurses for accurate medical documentation.
• Document treatment plans and goals to help clients achieve independence.
• Attend meetings to discuss progress and gather input.
• Provide transportation as needed for emergencies.
• Mentor new staff by guiding routines and training materials.
• Perform nightly sleep checks and participate in emergency evacuations.
