Job summary

Qualifications

• High school diploma or equivalent is preferred.
• Must hold a valid NYS driver's license at all times.
• Ability to understand and follow written and oral directions.

Responsibilities

• Provide ongoing guidance and support to individuals to promote independence and wellbeing.
• Assist with self-care routines, health, safety and meals as needed.
• Transport to medical appointments, shopping and activities; document every interaction.
• Support participation in community activities and attend inter-disciplinary meetings as required.
• Follow treatment plans, supervise daily living activities, and maintain records.
