Citizens Options Unlimited seeks a Direct Support Staff member to assist individuals in the Residential Services program, promoting quality of life through daily living support, health and safety, self-care, and social engagement. You will follow care plans, document progress, and work with an interdisciplinary team to enable independence and community participation.

NYS driver's license and strong communication are required; a high school diploma or equivalent is preferred, with POM training

Qualifikationen

• Effective written and verbal communication is required.
• Must be able to follow instructions and document care.
• Must have or obtain a valid NYS driver’s license and maintain status.

• Provide ongoing guidance and support to individuals to ensure active participation in daily life.
• Assist with self-care routines, health, safety, and household chores.
• Support socialization and community engagement; follow treatment plans.
• Document all plans, notes, and progress; participate in team meetings.
• Transport individuals to medical appointments, errands, and activities.
• Mentor new staff and ensure training compliance.
• Adhere to scheduling, staffing policies, and safety protocols.
