Job summary

• Mathematical skills to apply fractions, percentages, ratios, and proportions.
• Reasoning ability to solve practical problems with concrete variables.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical, nursing staff, and other departments to plan social services.
• Develop and co-ordinate department policies and procedures.
• Maintain social service progress notes and communicate resident needs to the treatment team.
• Coordinate discharge planning and resource referrals with community agencies.
