Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating and directing the social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Assist in developing and implementing policies for identifying residents' social and emotional needs.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Provide information to resident/families on Medicare/Medicaid and financial assistance programs.
• Coordinate social service activities with other departments.
• Maintain confidentiality of resident and facility records.
• Attend in-service education programs.
