Responsibilities

• Be a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Position Details

• Location: North Bay Hospital - New Port Richey, FL
• Status: PRN, As Needed
• Schedule: Days Vary, 8:30 AM - 5 PM
• Weekend requirement: Occasional
• On Call: No

Certifications And Licensures

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

Education

• Required: Masters in Social Work, Sociology, or Related Field
• Preferred: Masters Degree in Social Work

Experience

• Required 6 months Internship or
• Required 1 year Social Work in an acute care or hospital setting
