Job summary

Pepperbush Holdings, LLC (San Diego Post Acute) is seeking a social services professional to assist in planning, developing, and directing social service programs within the facility.

You will collaborate with administration, medical and nursing staff, and other departments to address residents' social, emotional, and financial needs, maintain progress notes, and support discharge planning.

Qualifications

• Must possess, at minimum, a high school diploma or its equivalent.
• Associate or Bachelor’s Degree preferred but not required.
• Prior health care experience preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility.
• Meet with administration, medical and nursing staff, and other related departments in planning social services.
• Assist in developing and coordinating department policies and procedures.
• Review department policies and procedures, at least annually, and participate in making recommended changes.
• Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
• Coordinate social service activities with other departments as necessary.
• Assist residents in achieving the highest practicable level of self care and well being.
• Provide information to residents/families as to Medicare/Medicaid and other financial assistance programs.
