BayCare Health System invites a Social Services Specialist II to join Morton Plant Hospital in Clearwater, FL. This full-time role involves assessment, coordination, and discharge planning with a collaborative care team to support patients and families through transitions.

The ideal candidate holds an MSW or related field, has 6 months internship or 1 year hospital social work experience, and prefers CCM/ACM credentials. Benefits include health coverage, PTO, 401k, and tuition reimbursement.

The Social Services Specialist Responsibilities Include

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Position Details

• Location: Morton Plant Hospital - Clearwater, FL
• Status: Full time, 40 hours per week. Exempt.
• Schedule: Monday - Friday 8:00 AM - 4:30 PM
• Weekend requirement: Occasional
• On Call: No

Certifications And Licensures

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

Education

• Required Masters in Social Work, Sociology, or Related Field
• Strongly

Experience

• Required 6 months Internship or
• Required 1 year Social Work in an acute care or hospital setting
