Humanitary Medical Center Tampa, Inc. is seeking an experienced Social Services Coordinator to guide patients on social rights, benefits, and access to community resources.

The role requires coordinating assessments, liaising with providers, and ensuring adherence to care plans in a fast-paced environment. The ideal candidate will be bilingual (English/Spanish), have strong communication skills, and experience in acute or social services settings.

Qualifications

• Minimum of High School diploma or equivalent combination of education and experience.
• Experience in acute care, managed care, or social services environment preferred.
• Proficiency in Microsoft Office; knowledge of EHR software.
• Bilingual in English and Spanish is required.
• Excellent customer relations and communication skills.

Responsibilities

• Provide information and guidance to patients about social rights, benefits, and resources.
• Coordinate home assessments with members and families.
• Educate enrollees about programs including Community Based Services.
• Act as liaison between health plans, providers, enrollees, and families.
• Identify gaps or barriers in treatment plans and assist members accordingly.
• Coordinate resources to meet patient needs and ensure ongoing care.
• Follow up with members to verify treatment adherence and services received.
• Maintain confidentiality and comply with HIPAA, OSHA, and related regulations.
