PACS is seeking a Social Services professional to assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs in line with federal, state, and local standards. Qualifikationen

• High school diploma or equivalent; Associate or Bachelor’s degree preferred.
• Prior health-care experience preferred.
• Ability to read, write and communicate effectively.
• Knowledge of Medicare/Medicaid programs and benefits.

• Assist in planning and directing social service programs.
• Meet with admin and staff to plan services.
• Develop and coordinate department policies and procedures.
• Provide information on Medicare/Medicaid to residents and families.
• Record social service progress notes and report to Director.
• Coordinate with other departments to maintain services.
