Job summary

Volunteers of America Colorado is seeking a Service Coordinator for the Westgate Towers Apartments to manage housing services for residents at an affordable housing site. The role emphasizes trauma-informed care, harm reduction, and Motivational Interviewing to help residents age in place and maintain independence.

You will conduct needs-based assessments, provide case management, support outreach, and connect residents to income resources and public benefits.

Qualifications

• Bachelor’s degree in human services, social work, or related field.
• At minimum, one full year of working with senior citizens and/or people with disabilities.
• Willing to obtain first aid, AED and CPR certifications and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal skills and good organizational skills.
• Proficient in using Windows environment and working with databases.

Responsibilities

• Ensure resident voices are heard and included in programming.
• Accept referrals from the Community Administrator, family, providers, and residents.
• Assess services needed by residents and maintain accurate records.
• Coordinate and monitor services to ensure quality of care and cost effectiveness.
• Maintain up-to-date provider files and report required information.
• Develop and facilitate support groups and workshops as needed.
