Job summary

Volunteers of America Colorado is seeking a Housing Case Manager to deliver direct housing support and case management at the Lakewood Navigation Center. You will assess needs, connect guests to income and benefits, and coordinate with landlords and service providers to promote housing stability.

The role requires driving to programs, 40 hours/week, and adherence to best practices including trauma-informed care and crisis de-escalation. VOAC offers a comprehensive benefits package.

Qualifications

• Bachelor’s degree required in social work or a closely related field or equivalent experience.
• Solid written and verbal communication and strong organizational skills.
• Experience serving individuals experiencing homelessness is preferred.

Responsibilities

• Conduct needs-based assessments and determine program eligibility for housing services.
• Assist participants in securing and maintaining housing and communicate with landlords.
• Coordinate outreach with community providers and multidisciplinary teams.
• Maintain accurate records in HMIS and Service Point and ensure timely reporting.
• Assist with program development and crisis intervention as needed.
