Job summary

VOAC Colorado is seeking a Service Coordinator to oversee service provision and housing assistance for residents in the Affordable Housing Portfolio. This program serves seniors and persons with disabilities located at affordable housing sites.

The role uses Trauma Informed Care, Harm Reduction, and Motivational Interviewing, and works with the Community Administrator to help residents live independently and age in place.

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field.
• At minimum, one full year of working with senior citizens and/or people with disabilities.
• Willing to obtain first aid, AED and CPR certifications and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal skills and good organizational skills.
• Proficient in Windows environment and working with databases.

Responsibilities

• Ensure residents' voices are heard and incorporated into programming.
• Accept referrals from the Community Administrator, family, providers, and residents.
• Assess resident needs via surveys, visits, and meetings; collect information.
• Deliver services within modalities using an equity lens.
• Provide residents information on specific services and providers with multiple options.
• Encourage residents to arrange required services to maintain independence.
• Coordinate and monitor providers to ensure quality and cost-effectiveness.
• Maintain up-to-date provider files and use United Way and Benefits Check-up databases.
• Maintain accurate resident records and service histories.
• Develop and facilitate support groups and workshops.
• Relocate residents as needed and secure emergency housing.
• Support team with problem-solving and stay current on field practices.
• Implement the Service Coordinator program per HUD and VOA guidelines.
• Attend trainings, meetings, and may mentor others.
