Job summary

DaVita is seeking a qualified RN to coordinate and facilitate quality, cost-effective care for ESRD and CKD patients across the care continuum. You will work autonomously and with interdisciplinary teams, traveling 3–4 days per week within your territory to align care with patient needs and partner availability.

Responsibilities include reducing fragmentation of the healthcare system and guiding patients through complex transitions, with emphasis on data-driven CQI and Capella-based workflows.

Qualifications

• RN license current in state of practice.
• ADN required; BSN preferred; diploma may substitute.
• Minimum five years clinical nursing experience preferred.
• Renal nursing experience preferred (3+ years).
• Case management experience required or equivalent.
• Proficiency with Capella within 90 days.
• Strong data management and CQI knowledge.
• Driver's license with valid record.
• CPR certification current.

Responsibilities

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
• Minimize fragmentation of the healthcare delivery system
• Coordinate care among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager
• Work autonomously and collaboratively with all members of the healthcare team
• Travel 3–4 days per week within an assigned territory
• Accommodate patient needs and nephrology partner availability Monday–Friday
• Provide care management for medically complex and vulnerable kidney disease populations
