Job summary

Somatus is seeking an RN Care Manager (VBC) to support high‑needs CKD/ESKD populations. You will partner with nephrology practices, coordinate home visits, and educate patients while developing individualized care plans. Travel within the region is required, with a focus on reducing readmissions and improving outcomes.

You will work with interdisciplinary teams, supervise non‑licensed staff, and ensure adherence to care plans, patient education, and HIPAA compliance across multiple settings.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in multidisciplinary teams with physicians.
• Valid BLS certification from an approved facility.
• Renal, CKD or dialysis care experience preferred.
• Willingness to travel within assigned region for visits.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Plan, coordinate, and facilitate interdisciplinary care team meetings.
• Build relationships with patients and provider offices face-to-face and via telecommunication.
• Collaborate with practice teams to optimize workflows and care processes.
• Educate patients on discharge care requirements and renal conditions.
• Provide clinical guidance to non-licensed and licensed team members.
