Job summary

Somatus seeks an RN Care Manager, VBC to support high-need CKD/ESKD patients. You will partner with nephrology practices to deliver care coordination, education, and outcomes-focused support across home, clinic, and hospital settings.

You will travel within the assigned region to conduct visits, perform assessments, and implement individualized care plans with a patient-centered approach.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience within a multi-disciplinary team and with physicians.
• Valid BLS certification from a licensed training facility.
• Renal, CKD or Dialysis care experience as main focus of your job.

Responsibilities

• Collaborate with physicians and practice staff to improve patient outcomes and serve as the primary Somatus representative within the practice.
• Coordinate and participate in regular interdisciplinary care team meetings with partner practices.
• Establish and maintain positive relationships with patients and provider offices through in-person and telephonic engagement.
• Develop partnerships with provider practice teams to support clinical and operational goals.
• Educate provider practices on the Somatus program and reinforce integrated workflows.
• Provide complete care coordination across in-person, telehealth, or phone interactions, including assessments and care plans.
• Travel to member homes, facilities, and physician offices for visits and care planning.
• Manage transitions of care and minimize preventable readmissions.
