Job summary

Somatus is seeking an RN Care Manager to support high-need CKD/ESKD patients through field-based and telehealth visits. You will collaborate with nephrologists, PCPs, and care teams to deliver coordinated, person-centered care and remove barriers to treatment.

You’ll conduct in-home assessments, develop care plans, educate patients, and help reduce readmissions while navigating complex care pathways. Travel is required within the designated market.

Qualifications

• Active RN license and eligibility for multi-state licensure as needed.
• Minimum 2 years RN experience in multidisciplinary teams.
• BLS certification from an approved training facility.
• Renal, CKD, or dialysis care experience preferred as a focus.
• Willingness to travel within the assigned region for patient care.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Coordinate regular interdisciplinary care team meetings with partner practices.
• Establish and maintain relationships with patients and provider offices.
• Develop and implement individualized care plans with the nephrologist and care team.
• Travel to member homes, clinics, and facilities to conduct visits and care planning.
• Guide non-licensed and licensed team members in clinical pathways and workflows.
