Job summary

DaVita Integrated Kidney Care in El Paso, Texas seeks an experienced Nurse Case Manager (RN) to coordinate ESRD/CKD patient care and collaborate with interdisciplinary teams. You will support field-based case management across El Paso and surrounding areas, traveling as needed to meet patient needs.

Current CPR and Capella proficiency are required. Ideal candidates have ADN; BSN preferred, five years of clinical nursing experience, and three years in renal nursing.

Responsibilities

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
• Minimize fragmentation of the healthcare delivery system
• Coordinate care among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager
• Function autonomously and collaborate with all members of the healthcare team
• Provide field-based case management across El Paso, surrounding areas, and other assigned regions
• Travel within the assigned territory
• Support DaVita Integrated Kidney Care’s mission to improve patient quality of life, clinical outcomes, and total cost of care

Requirements

• A.D.N degree from an accredited school of nursing required; B.S.N preferred
• Three-year diploma from an accredited diploma program may be substituted for a nursing degree
• Current Registered Nurse (RN) license in the state of practice required
• Continuing education credits maintained as required by state of practice required
• Minimum of five (5) years’ experience in clinical nursing strongly preferred
• Minimum of three (3) years’ experience in renal nursing preferred
• Case management experience required
• In lieu of case management experience, 3 or more years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions required
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
• Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
• Demonstrated experience and effectiveness in change agent role
• Demonstrated knowledge and understanding of CQI techniques
• Previous experience in healthcare performance coaching required
• Current CPR certification required
• Ability to modify personal practice patterns to adapt to new/electronic processes and increased productivity expectations as it pertains to Capella implementation
• Current driver’s license in state employed with positive driving record and ability to meet insurance coverage requirements required
• Basic computer skills and proficiency in MS Word and Outlook required
• Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment
• Home office with internet connectivity of at least 1MB upload and 1MB download speed required
• Ability to travel within an assigned territory
• Flexibility to accommodate patient needs and nephrology partner availability
