Job summary

DaVita Integrated Kidney Care in El Paso, Texas seeks an experienced Nurse Case Manager (RN) to coordinate ESRD/CKD patient care and collaborate with interdisciplinary teams. You will support field-based case management across El Paso and surrounding areas, traveling as needed to meet patient needs.

Current CPR and Capella proficiency are required. Ideal candidates have ADN; BSN preferred, five years of clinical nursing experience, and three years in renal nursing.

Qualifications

• RN license in the state of practice is required.
• Five years of clinical nursing experience preferred.
• Three years in renal nursing preferred.
• Case management experience required.
• CNN or CCM certification preferred.
• Must be able to work with electronic health records and Capella.
• Current CPR certification required.

Responsibilities

• Coordinate quality, cost-effective care for ESRD and CKD patients.
• Minimize fragmentation of the healthcare delivery system.
• Coordinate care among patients, families, physicians, providers, care teams and regional managers.
• Function autonomously and collaborate with healthcare team.
• Provide field-based case management across El Paso and surrounding areas.
• Travel within assigned territory.
• Support DaVita Integrated Kidney Care mission to improve patient outcomes and satisfaction.
