Job summary

U.S. Renal Care seeks a Value-Based Care Nurse Case Manager to collaborate with patients and care teams in Texas. The role focuses on education, care coordination, and proactive management to reduce hospitalizations and support dialysis/transplant planning.

Responsibilities include case management, education, and working with nephrologists to optimize care. On-site work in Texas with a patient-centered, compliant approach to CKD/ESKD care.

Qualifications

• RN license in Texas (current and in good standing).
• 3+ years nursing experience preferred.
• Experience in case management and/or providing nursing care to dialysis patients preferred.
• Spanish bilingual required (written and verbal).
• Proficiency with USRC clinical applications within 90 days of hire.

Responsibilities

• Assess, implement, coordinate, monitor, and evaluate patient care.
• Provide health education, coaching and treatment decision support.
• Collaborate with patients, caregivers and other providers to address needs.
• Build partnerships with nephrologists and dialysis team to manage care.
• Drive metrics to reduce hospitalizations and improve dialysis starts.
• May perform outbound calls and in-person visits to manage care.
• Ensure compliance with applicable laws and company policies.
• May work on-site with care team in Texas; full-time position.
