Job summary

DaVita Integrated Kidney Care (DaVita IKC) is seeking an RN Case Manager to coordinate care for ESRD and CKD patients across the Tacoma area. This hybrid, field-based role requires a dedicated home office and 3–4 days of travel each week to visit patients within the assigned territory.

You will collaborate with patients, families, physicians, providers and care teams to deliver high-quality, cost-effective care, minimize fragmentation, and support transitions through the care continuum.

Qualifications

• A.D.N. degree required; B.S.N preferred; three-year diploma may substitute for nursing degree.
• Current RN license in the state of practice required.
• Minimum of five years of clinical nursing experience.
• Minimum of three years of renal nursing experience preferred.
• Case management experience required; if not, 3+ years coordinating care across teams.

Responsibilities

• Coordinate and facilitate care for ESRD and CKD patients.
• Collaborate with patients, families, physicians, providers and care teams to deliver high-quality, cost-effective care.
• Serve as liaison across community and Regional Operations Manager and care teams.
