Job summary

Somatus is seeking an RN Care Manager, VBC, focusing on high-need CKD/ESKD populations. The role involves outreach, scheduling home visits, dialysis clinics, and physician offices to support high-risk members and improve outcomes.

You will collaborate with nephrologists, PCPs, and the care team to develop individualized care plans, remove barriers to care, and coordinate transitions. Travel within the assigned market is required, with field-based and remote work options available.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and physicians.
• Valid BLS certification ONLY from a licensed facility.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
• Reside in a location with high-speed internet or can leverage existing service.
• Comfortable traveling to partner hospitals, clinics, and community facilities within your assigned region.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Support care team in planning, coordinating, and facilitating regular interdisciplinary care team meetings.
• Establish and maintain relationships with patients and provider offices.
• Develop partnerships with provider practice teams to support clinical and operational goals.
• Collaborate to optimize workflows that align with care team processes.
• Educate provider practices on the Somatus program and integrated workflows.
• Provide care coordination through in-person, telehealth, or phone interactions; conduct assessments and care planning.
