Job summary

Somatus is seeking an RN Care Manager to support high-need CKD and ESKD patients across our network. You will work with patients, physicians, and health plans to coordinate care, assess barriers, and implement individualized care plans.

You’ll travel to member homes, dialysis clinics, and physician offices to deliver coordinated care, educate teams, and ensure seamless transitions. This field-based role requires strong teamwork and communication.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in multidisciplinary settings and physicians.
• BLS certification from a licensed facility.
• Renal, CKD or dialysis care as a primary focus.

Responsibilities

• Partner with physicians and practice staff to build trusted relationships.
• Assist in planning and coordinating regular interdisciplinary care team meetings.
• Establish positive relationships with patients and provider offices through in-person and telephonic engagement.
• Develop partnerships with provider practice teams to support clinical and operational goals.
• Collaborate with practices to optimize workflows aligned with care objectives.
• Educate provider practices on the Somatus program and integrated workflows.
• Deliver complete quality care via in-person, telehealth or remote interactions, including assessments and reassessments.
• Travel to member homes, clinics and offices to conduct visits and care planning.
• Use nursing assessment skills to identify barriers to care and address needs.
• Develop individualized care plans with the nephrologist and care team to improve outcomes.
• Manage transitions of care and minimize preventable readmissions.
• Provide clinical guidance to non-licensed and licensed team members as needed.
• Perform other duties as assigned.
