Job summary

Somatus seeks an RN Care Manager, VBC focused on high-needs CKD and ESRD populations. You will work with patients, nephrologists, PCPs, and interdisciplinary teams to schedule visits, educate patients, and develop individualized care plans in home, clinic, and facility settings.

You will travel within your region, build trusted practice relationships, and support care coordination to improve outcomes while ensuring continuity of care across settings.

Qualifications

• RN license required; ability to obtain licensure in other states
• 2+ years RN experience in multidisciplinary team settings
• BLS certification from a licensed facility
• Renal, CKD or dialysis care experience preferred
• Willingness to travel locally to partner hospitals, clinics, and community facilities
• Field-based/remote work adaptability and strong communication skills

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes
• Coordinate regular interdisciplinary care team meetings with partner practices
• Establish and maintain relationships with patients and provider offices
• Develop workflows with provider teams to align clinical and operational goals
• Educate provider practices on the Somatus program and integrated care workflows
• Provide comprehensive care coordination and education to patients and families
• Travel to member homes, clinics and offices for visits and care planning
• Manage transitions of care and prevent readmissions
• Supervise non-licensed and licensed team members as needed
• Perform other duties as assigned
